The following tools are offered to help you evaluate your own mental health and mental wellness:

- Mental Health Continuum Model
- Life Hub Health Risk Appraisal
These tools will provide you with feedback about your own mental health and mental wellness, along with tips for improving your mental wellness.

Mental Health Meter
Assessing our mental health is not as simple to do as measuring our physical health. There are no scales or endurance tests that rate mental fitness. However, with the help of the Canadian Mental Health Association’s Mental Health Meter, you can reflect on your unique strengths and identify areas where your level of mental fitness could be improved.
The Mental Health Meter uses characteristics that make up good mental health to help us determine how mentally fit we are.
These characteristics include:
- The ability to enjoy life
- Resilience
- Balance
- Self-actualization
- Flexibility
Working Mind Mental Health Continuum Model
The Working Mind Mental Health Continuum Model categorizes mental health along a continuum using the following colours:
- Green (healthy)
- Yellow (reacting)
- Orange (injured)
- Red (ill)
The purpose of the continuum:

It allows individuals to identify indicators of declining or poor mental health in themselves and others (without diagnostic labels and their associated stigma).

It stresses that individuals can move along the continuum. For example, if one ends up in the red (ill) phase, they're still able to move back towards the green (healthy) phase.

It teaches appropriate actions one can take for themselves and for others at each point along the continuum.
LifeHub: Health Risk Appraisal
LifeHub is offered to PHC staff through the Employee and Family Assistance Program (EFAP) and contains health and wellness information and self-help tools.
When you sign up, you will be asked for a key code. Enter "PHC" where prompted. Please note: any information entered into LifeHub is confidential.
LifeHub includes an online goal-setting and tracking system, and a built-in Health Risk Appraisal that covers a range of work/life balance topics, including:
- Stress
- Health, exercise and nutrition
- Sleep
- Happiness and spirituality
- Infants and children
- Relationships
- Career management
- Leadership
- Addictions, depression and anxiety
- Personal finance
- Elder care
